DOT PHYSICAL FOR CDL LICENSE

We perform a physical exam that meets DOT and Federal Motor Carrier Safety Administration (FMCSA) requirements for a commercial driver’s license (CDL). All commercial motor vehicle drivers on the road are required to take a series of DOT physical exams throughout their career. Dr. Levine, a chiropractic physician and certified medical examiner, will perform these tests. A DOT physical exam is valid for up to 24 months.

PHASE ONE OF YOUR EXAM:

MEDICAL HISTORY
Phase one will cover the last five years of the individual’s medical history including any injuries, surgeries or illnesses. A list of any medications taken regularly and any special medical conditions will be reviewed with the doctor.

PHASE TWO OF YOUR EXAM:

VISION & HEARING
Vision & Hearing will be phase two of the exam. The individual will undergo a routine eye exam and a hearing test that will be performed by the physician.

PHASE THREE OF YOUR EXAM:

BLOOD PRESSURE & URINALYSIS
In phase three, the individual’s blood pressure will be tested and a urine sample will need to be taken. A blood pressure of less than 140/90 is considered normal by the Department of Transportation. The urinalysis will test for abnormalities in sugar levels, proteins, white or red blood cells and specific gravity (hydration).

PHASE FOUR OF YOUR EXAM:

PHYSICAL & NEUROLOGIAL TESTING
Phase four consists of a physical & neurological examination. The physical exam will check all organ systems. The neurological exam will cover the reflexes, balance and coordination of the individual.

This physical requires payment (cash, credit, debit or personal check) upon completion of this examination and will not be submitted to your insurance carrier by our office. A receipt will be given to you for your records or to provide to your insurance company if you choose to do so.
