Insurance Information

CHIROPRACTIC HEALTH CENTRE accepts most insurance plans including auto accidents and worker's compensation carriers, Horizon BCBS, Medicare, Aetna, Cigna, United Health Care and many others.

Just bring your card or information and we will do the processing and billing. Our intent is to make insurance billing as hassle-free as possible for our patients.

Verification of insurance benefits can be obtained prior to coming in for an initial office visit by contacting the 800 phone number on your insurance card. As a courtesy, we will call to verify benefits with your insurance company.

Plans will vary depending on agreements your employer has with the insurance carrier. However, the benefits quoted to us by your insurance company are not a guarantee of payment. Payment may be due by you at the time of service for any non-covered services, deductibles or co-pays. We encourage all patients to contact your insurance company to verify coverage prior to your visit.

  • “ON THE JOB” INJURY (Workers’ Compensation)
    If you are injured on the job, your care should be paid for under your employer’s Workers’ Compensation insurance. You will need to inform your employer of the accident, fill out an accident report and obtain the name and address of the carrier of their insurance. All treatment in our office needs to be pre-authorized in writing by your employer or insurance company.
  • PERSONAL INJURY OR AUTOMOBILE ACCIDENTS
    Please notify your own auto insurance carrier of your visit to our office immediately. Your medical benefit on your auto insurance is billed first regardless of who was at fault in the accident or whose automobile you were in. Notify our office immediately if an attorney is representing you. This way our office has ongoing dialogue with your attorney with regard to the status of your condition and case. If there is any unpaid balance on your bill, we will wait for settlement of your claim after your care is completed. Once the claim is settled, or if you suspend or terminate care, any fees for services are due immediately.
  • MEDICARE
    We do accept assignment from Medicare. The check is usually sent directly to our office for payment of the services that Medicare will cover, which for chiropractors is ONLY manual manipulation of the spine. Medicare pays 80% of the allowable fee once the deductible has been met. You are required to pay any non-covered services. Our office completes and submits the forms to Medicare, on your behalf, at no charge.
  • SECONDARY INSURANCE
    Please inform us of any secondary insurance you may have. Secondary insurance is a supplemental insurance you may have by yourself, spouse or domestic partner. Our office completes and submits all secondary insurance at no charge.
  • NO INSURANCE COVERAGE
    Please do not let your insurance coverage (or lack of insurance) dictate the treatment you need to correct and maintain your health. If finances are an obstacle, please discuss this with us. We offer payment plans to fit almost any budget.
